If your family income is $100,000 a year then you are good to go to submit your online application free of cost by using the fee waiver. If you are not eligible for a fee waiver then you will have to pay $25 application fee for the initial application.

  • Your federal income tax returns of last two years.
  • W-2 forms and other records of current year income
  • Records of untaxed income and benefits, assets.
  • Current bank statement
